Please Note: Before your initial appointment you must complete a health history and have a medical consultation with our medical director, Dr. Ivan Ramirez. The doctor determines your eligibility during the medical clearance process. Upon successful clearance, you may receive the requested service. The prescriptive order clears you for all medical grade services offered at Rejuvalight Aesthetics for one year unless there are significant changes to you health status. If you haven't connected with the doctor a minimum of 48 hours prior to your scheduled treatment, your appointment will need to be rescheduled. The medical consult is required by the Texas Medical Board; TMB Rule; 193.17.
Treatments are by appointment only. A valid credit card is required to reserve your appointment, even if you have prepaid for your session. Your card will not be charged unless there is a late cancellation or not showing for your scheduled service(s) as per our cancellation policy.
Your appointments are very important to us and are reserved especially for you. Should you need to cancel or reschedule your appointment, we respectfully request at least a 24 hour notice. Failure to show up to a scheduled appointment will result in a charge for $50 dollars from the credit card on file. Cancellations inside the 24 hours prior to the appointment may result in a charge, upon our discretion, that can be used toward a rescheduled service.

The medical consultation with our medical director is not a substitute for a complete dermatological evaluation. We do not diagnose or cure any type of skin disease or medical condition. Pre-existing medical conditions require your physician’s permission.
